Service Coordination / Case Management
Our Service Coordinators/Case Managers provide families and children with day-to-day coordination and support. They are responsible for reviewing clinical reports, assessing the strengths and needs of family members and consumers, and developing a plan for service. Collaborating with existing behavioral health providers, as well as other community and local resources, traditional and non-traditional, our team assists families in implementing their service plan. Our Service Coordinators/Case Managers are fully trained in a variety of areas such as NHTD/TBI (Nursing Home Transition and Diversion/Traumatic Brain Injury), Medicaid Eligibility, OPWDD, and more. Whatever the need may be, we are here to help. From Food Stamp Assistance and HRA to Public Assistance, Medicaid Applications, Fair Hearing Services (as needed), Housing Assistance, or just guidance, our team is trained to assist families in all areas of need.
